LIFTING

Seven lifting devices, chosen by skin layer

Give Me Clinic runs seven lifting devices. Sofwave covers the epidermis. Thermage, Volnewmer and Sofwave cover the dermis. Onda Pro and Ultherapy Prime cover the subcutaneous fat and the fascia. The ultrasound devices apply energy at three separate depths: 1.5mm, 3.0mm and 4.5mm. The radiofrequency devices deliver heat across the dermis. The examination establishes which layer has loosened and how thick the fat layer is. We then set the device, the dose and the interval between sessions.

ONDA PRO

Onda Pro

Onda Pro delivers heat to the fat layer and the dermis with microwaves. It uses three tips: 3mm, 7mm and 12mm. One session takes about 20 minutes per area, and no anaesthesia is used.

THREE TIP SIZES

The tip is chosen to fit the area

A wide tip for broad areas, a small tip for narrow, deeper ones. The 3mm, 7mm and 12mm tips cover different depths and different amounts of ground.

  • 12mm, wide areasThe tip with the widest contact surface. It is used on large areas: the abdomen, the thighs and under the jaw. It covers more ground in one pass, so the same area takes less time.
  • 7mm, deep fat layerHeat reaches as far as the deep fat layer. It encourages fat cells to break down, and works on the collagen fibres beneath to stimulate fibroblasts.
  • 3mm, near the surfaceEnergy goes into the collagen fibres close to the surface. We use it at the stage that tidies surface texture, once the deeper layer has been treated.
  • Areas beyond the faceThe same principle applies to the arms, abdomen and thighs. We work out the application time and the number of sessions again from the size of the area.
  • 3-5 sessions, 2-4 weeks apartRather than one strong session, treatment is split into three to five. Between sessions we check whether redness and warmth remain, then set the intensity for the next one.

EARLY ADOPTER

Among the earliest clinics in Korea to bring in Onda Pro

Give Me Clinic was among the earliest clinics in Korea to bring in Onda Pro, and has run it ever since. Onda Pro makes heat by vibrating the water molecules inside tissue with 2.45GHz microwaves. Unlike radiofrequency or ultrasound, the heat also reaches fat, which holds little water. That lets us work on an area where fat has built up, such as a double chin, and on the dermis above it in the same session. No anaesthesia is used, and one area takes about 20 minutes. The dosing and the session intervals we use for each area are the ones built up since the device arrived.

VOLNEWMER

Volnewmer

A 6.78MHz monopolar radiofrequency device made by Classys. It delivers heat across the dermis. The procedure takes about 20 minutes and is carried out under topical anaesthesia. The result holds for up to about 12 months.

MONOPOLAR RF

6.78MHz radiofrequency coagulates dermal collagen

Volnewmer is a 6.78MHz monopolar radiofrequency device made by Classys. It delivers heat into the dermis to coagulate collagen tissue, and new collagen forms as that site heals. The tip edges are curved so the contact surface stays flush against the contours of the face. Cooling has six levels and vibration five, both adjustable during the session. We read how your skin responded to the first session, then reset the energy level and the number of pulses for the next.

TIP CONTACT AREA

The tip changes with the area

The number engraved on each tip is the size of its contact surface. An angled area such as the jawline and a narrow area such as around the eyes take tips with different contact areas.

  • V TIP 4.0
V TIP 4.0Used on narrow, angled places such as the jawline and where it meets the neck. The contact surface is shaped to stay down along a curve, so the energy reaches evenly as we work along a line.
  • I TIP 0.25
I TIP 0.25The tip with the smallest contact surface. It is used around the eyes, where the skin is thin and bone is close. Narrowing the area keeps the heat to the points that need it.

THERMAGE

Thermage

A monopolar radiofrequency device that delivers heat to the dermis. The procedure takes about 20 minutes and is carried out under topical anaesthesia. We do not stack sessions. After one application we check how long the result holds, which is up to about 12 months.

BULK HEATING

Heat across the dermis, cooling at the surface

Thermage delivers heat across the whole area being treated. We do not repeat it at short intervals. After one application we watch the two to three months in which collagen forms.

  • Applied across the dermisRadiofrequency is delivered across the dermis to coagulate collagen. It passes over the whole treatment area rather than a single point, so we use it when the contour has become heavier overall.
  • Cooled as it is appliedThe tip cools the surface the moment the energy is delivered. Heat stays in the dermis while the temperature of the epidermis comes down. We raise or lower the intensity during the session according to how much it hurts.
  • Assessed after 2-3 monthsWe judge the result from how things look two to three months later, not straight after. Whether to repeat is decided from how long the result held, never simply because time has passed.

THERMAGE FLX

Radiofrequency applied while the tip reads temperature

The Thermage FLX unit and handpiece. The tip reads the surface temperature and applies cooling and vibration while radiofrequency heat goes into the dermis. We adjust the intensity to how the treatment feels as we go.

SOFWAVE

Sofwave

A device that creates heat at a depth of 1.5mm using ultrasound emitters set side by side. The procedure takes about 30 to 45 minutes and is carried out under topical anaesthesia. The result holds for about three to six months, and we examine again at intervals of about three months.

HEAT AT 1.5MM

Heat zones at 1.5mm

Sofwave does not overlap in depth with the devices that reach the fascia at 4.5mm. That is why it can sit alongside Ultherapy Prime in one plan, each taking a different layer.

  • Emitters set side by sideRather than focusing ultrasound on one point, several emitters sit side by side and create heat zones at the same depth. The area covered is shallower and wider than with a single focal point.
  • Applied at 1.5mmIt works at 1.5mm, the upper dermis, just beneath the epidermis. That depth does not overlap with the devices that reach the fascia at 4.5mm.
  • Surface cooled during applicationThe contact surface cools the epidermis while the energy is applied. Redness can remain for a few hours afterwards, so we go through moisturising and sun protection with you as well.

ULTRASOUND ARRAY

An ultrasound device with emitters set side by side

The Sofwave unit and applicator. A handpiece of parallel ultrasound emitters creates heat zones at 1.5mm, and the contact surface cools the epidermis while the energy is applied.

ULTHERAPY PRIME

Ultherapy Prime

A device that gathers ultrasound to form thermal coagulation points at three depths: 1.5mm, 3.0mm and 4.5mm. The procedure takes about 20 minutes and is carried out under topical anaesthesia. The result holds for about six to twelve months.

HIFU

Ultrasound gathered as deep as the 4.5mm fascia

Ultherapy Prime is a high-intensity focused ultrasound (HIFU) device. Ultrasound is gathered onto one point at a set depth, so thermal coagulation points form only there and no focus forms at the surface. 4.5mm is the fascia (SMAS), 3.0mm the fat and the fibrous septa, and 1.5mm the dermis. The three depths are applied separately. We check the layers on screen before we apply, because the thickness of the fat layer and the position of the fascia differ from person to person, so the same depth can reach different tissue.

EpidermisDermisSubcutaneous fatSMAS1.5mm3.0mm4.5mm

TREATMENT DEPTH

Application depths: 1.5mm, 3.0mm, 4.5mm

The scale on the right of the diagram is depth measured from the surface of the skin. Sofwave works at one depth, 1.5mm. Ultherapy Prime works at three, 1.5mm, 3.0mm and 4.5mm, applied separately. 1.5mm is the upper dermis, 3.0mm the fat layer and the fibrous septa, and 4.5mm the fascia (SMAS). The ellipse at the end of each of the three arrows running down from the surface is where the focus forms at that depth. Even at the same 4.5mm, the energy does not reach the fascia if the fat layer is thick. So we check the layers on screen before we apply, and choose the depth again.

FACE INMODE

Face InMode

A device that works on shallow, narrow facial fat with radiofrequency and electrical stimulation. It takes the places that blur the facial line, such as a double chin and deep cheek fat. Two modes, FX and FORMA, are used separately, and the output is adjusted while the temperature of the epidermis is measured in real time.

InModeInModeView equipment details

DUAL MODE

FX and FORMA, two modes on one unit

The InMode platform we run at the clinic. The FX and FORMA applicators sit on one unit and we switch between them by area. FX mode delivers radiofrequency and electrical stimulation to the fascia and fat layers, taking narrow, thicker places such as the cheeks, deep cheek fat and the double chin. FORMA mode delivers heat to the dermis and epidermis to prompt collagen production. The output is adjusted while the temperature of the epidermis is measured in real time. Devices that reach the same depth go into separate sessions rather than the same day.

SHURINK UNIVERSE

Shurink Universe

A focused ultrasound device that gathers ultrasound onto a single point. Changing the cartridge changes the depth it reaches. That depth overlaps with Ultherapy Prime, so the two are not used on the same area. Which one we use is decided from the size of the area and the record of previous treatment.

MICRO FOCUSED

Cartridge and mode, chosen for the area

Even among focused ultrasound devices, the use differs with the depth and the way the energy is applied. Shurink Universe widens that range by changing cartridge and mode.

  • Cartridges set the depthChanging the cartridge changes the depth at which the focus forms. We choose the cartridge to match the layer found at the examination.
  • Point by point, or along a lineIt has a mode that applies point by point and one that applies along a line. We use them separately according to the size and shape of the area.
  • Kept apart from Ultherapy PrimeIts application depth overlaps with Ultherapy Prime, so the two are not used on the same area. Which one we use is decided from the size of the area and the record of previous treatment.

ONE FOCAL POINT

Focused ultrasound, gathered onto one point

The Shurink Universe unit. Changing the cartridge changes the depth at which the focus forms, and the dot and line modes are chosen to match the shape of each area.

PROCESS

This is the order your lifting follows

We build it up in stages rather than doing everything at once. The order and the intervals matter more than the number of devices, so treatment runs through five stages, from the examination to the review.

  1. 01We examine layer by layerWe look at the contour with your face relaxed and while you hold an expression, seated and lying down. Even with the same double chin, a thickened fat layer and a stretched fascia call for different devices.
  2. 02We set the target depthIf the fascia at 4.5mm is the cause, we use Ultherapy Prime. If the dermis is the cause, Thermage or Volnewmer. If the upper dermis at 1.5mm is the cause, Sofwave. If the fat layer is the cause, Onda Pro.
  3. 03We plan the order and the intervalsWe do not put several devices into the same area in one day. Devices whose application depths overlap are kept to separate periods. We confirm that redness and swelling from the previous treatment have settled, then move on to the next device.
  4. 04We adjust the dose by areaThe skin around the eyes is thin, so the dose there is lower than under the jaw. Where bone and nerves are close, the number of pulses is worked out separately.
  5. 05We review two or three months onNew collagen takes two to three months to form. So we see you again two to three months later, and decide about a further session from how things have progressed.

ANATOMY

Sagging starts in four layers, and each layer has its own device

1.5mm, SofwaveDermis, Thermage, VolnewmerFat layer, Onda Pro4.5mm, Ultherapy Prime
EpidermisDermisSubcutaneous fatSMAS
  • Epidermis, about 0.1-0.2mm thickThe outermost layer. Skin texture, fine lines and dryness show here. Lifting devices do not apply energy to it directly, but it is where change shows first after a treatment. Sofwave works at 1.5mm, just below it.
  • Dermis, about 1-3mm deepThe layer where collagen and elastin hold the skin firm. As it thins, pores stretch downwards and the skin starts to feel thin. Thermage and Volnewmer deliver radiofrequency heat across this layer, and Sofwave takes its upper part at 1.5mm.
  • Subcutaneous fat, below the dermisThe layer that gives the face its volume. A double chin is this layer thickened. A hollow cheek is this layer reduced. Onda Pro delivers heat into it through 3mm, 7mm and 12mm tips, and Ultherapy Prime works at 3.0mm.
  • Fascia (SMAS), about 4.5mm deepThe fibrous layer that ties the muscles of the face to the skin. When it stretches, the three layers above come down with it. Ultherapy Prime forms thermal coagulation points at 4.5mm, and Onda Pro reduces the weight of the fat layer sitting on top of it.

RECOMMENDED FOR

Start with an examination if any of these apply

The list below is what we go through when a consultation is booked. Which device we use is decided by the layer that has loosened. We advise once the examination has established the layer and the thickness of the fat.

  • If the jawline looks less defined than it used to
  • If a double chin or deep cheek fat still blurs your facial line
  • If the cheeks and the area around the mouth have dropped, making the face look heavier
  • If pores have stretched downwards and the skin feels thinner
  • If surgery feels like too much but you want to start working on firmness
  • If a long recovery is difficult and you need to be back in your routine the same day

AFTERCARE

Recovery after the procedure

  1. Day 0Straight afterDepending on the area there can be redness and slight swelling, which usually settles within a few hours. You can wash your face and use basic skincare, and go back to your routine the same day.
  2. Days 1-3A tender feelingThe treated area may feel achy or tender when pressed. Avoid saunas, hard exercise and alcohol, and pay more attention than usual to moisturising and sun protection.
  3. Weeks 1-2First changesA sense of tightening gradually settles in. How things look at this point is not the final result. Collagen renewal is only starting.
  4. Months 2-3Regenerative responseThis is when new collagen forms and is rearranged. We usually assess the result from how things look around this time, and talk through whether a further session is needed.
  5. From 6 monthsMaintenance and reviewHow long the result holds differs with the device, the area and daily habits. We examine again on the basis of about three to six months for Sofwave, up to about twelve months for Volnewmer and Thermage, and about six to twelve months for Ultherapy Prime.

FAQ

Frequently asked questions

There are several devices. Which one should I have?

It depends on which layer has loosened. Where the fat layer is thick we use Onda Pro. Where the dermis is the cause, Thermage or Volnewmer. Where the upper dermis at 1.5mm is the cause, Sofwave. Where the fascia at 4.5mm is the cause, Ultherapy Prime. Narrow, shallow places such as a double chin or deep cheek fat are worked on with InMode radiofrequency. We decide once the examination has established the layer, and treatment is often split across sessions rather than finished with one device.

Does it hurt a lot?

It differs with the device and the depth. Focused ultrasound gives a brief sharp feeling at the moment the heat gathers, and radiofrequency feels hot. Volnewmer, Thermage, Sofwave and Ultherapy Prime are carried out under topical anaesthesia. Onda Pro is carried out without it. Cooling and vibration bring the sensation down, and you can ask us to lower the intensity at any point during the session.

When does the change become visible?

A sense of tightening can be there from straight after the procedure. New collagen takes two to three months to form, so we judge the result from how things look two to three months later.

Why does the duration differ between devices?

Because the depth treated, and the rate at which that layer stretches again, differ. We quote about three to six months for Sofwave, up to about twelve months for Volnewmer and Thermage, about six to twelve months for Ultherapy Prime, and about six to twenty-four months for Onda Pro. Even with the same device it varies with the area, the condition of the skin and daily habits.

What side effects are there?

Temporary redness, swelling and tenderness when pressed are the most common. Bruising, burns, and sensory changes or weakness of the expression muscles from temporary nerve irritation have been reported rarely. Most settle with time, but please contact us straight away if any discomfort continues.

Can I combine this with other procedures?

We do not put several devices into the same area in one day. Ultherapy Prime and Shurink Universe in particular overlap in application depth, so they are not used in the same period. We confirm that redness and swelling from the previous treatment have settled, then move on to the next device.
